  1. Your current bank account numbers or most recent bank statement.
  2. A list of all direct deposits associated with your current account including payroll deposits, Social Security or any other income.
  3. A list of automatic debits or online bill payments such as utilities, mortgages, car payments, credit card companies, subscription services, Internet service providers, charitable organizations or other companies.
  4. Your personal information including proof of identity as required by law to open a new deposit account, your full name, date of birth and Social Security or Taxpayer Identification Number.
